Print, Complete, and Bring the require documents to a registration date.
Please bring 2 copies of the registration form with original signatures and both.

Requirements
- To participate your child must be 4 years old before August 1 and no older than 19 years before July 31.
There are NO exceptions.
- Current photo of each child cut to 1 X 1 inches (no larger or smaller) include child's name and birth date
on back of photo. Photos with hats or sunglasses will NOT be accepted.
NOTE: Passport pictures work great!
- NEW players must provide a copy of the child's birth certificate and is required at time of registration.
The copy will be kept for verification by league. If child played in last season, you are considered a
RETURNING player, and copy of birth certificate is NOT necessary.
- Registration Fee: $100.00 per player for Recreational and $125 per player for Metro/Select.
This fee includes a complete uniform, picture packet, and an end of season award.
