Right-click and select "Save Target As..." to download to your desktop. NOTE: Please bring 2 copies of the registration form with ORIGINAL signatures on each form. Requirements Registration is open to all youth who are at least 4 years old before August 1, 2010 and/or younger than 19 years old by July 31, 2010. There are NO exceptions to this. Current photo of each child cut to 1.5 X 1.5 inches (no larger or smaller) include child’s name and birth date on back of photo. Photos with hats or sunglasses will NOT be accepted. NOTE: Passport pictures work the best. New players must provide a COPY of the child’s birth certificate and is required at time of registration. Copy will be kept for verification by league. If child played in last season, 2009, you are considered a RETURNING player, and copy of birth certificate is NOT necessary. Registration Fee: $100.00 per player. Which includes a complete uniform, picture packet, and trophy. Registration forms will be provided at each date.
