Right-click and select "Save Target As..." to download to your desktop. Please bring 2 copies of the registration form with original signatures and both. Requirements Registration is open to all youth who are at least 4 years old before August 1 and/or younger than 19 years old by July 31. There are NO exceptions to this. Current photo of each child cut to 1.5 X 1.5 inches (no larger or smaller) include child’s name and birth date on back of photo. Photos with hats or sunglasses will NOT be accepted. NOTE: Passport pictures work the best. New players must provide a COPY of the child’s birth certificate and is required at time of registration. Copy will be kept for verification by league. If child played in last season, you are considered a RETURNING player, and copy of birth certificate is NOT necessary. Registration Fee: $100.00 per player. Which includes a complete uniform, picture packet, and trophy. Registration forms will be provided at each date.
